The author describes a process of psychoanalytic psychotherapy conducted in an institution with a severely compromised patient both internally and externally. The intervention is intented to reception and reconstruction of highly damaged patient’s areas; to the elaboration of psychic raw elements and to the integration of fragmented aspects of a story full of traumatic and agonizing events that are difficult to represent. The evolution of therapy shows a slow internalization of a minimal improved object, the construction of more solid internal aspects and mobilizes evident changes on external aspects with new invest-ment possibilities for the patient.
