The congenital anomalies of the central nervous system and the degenerative pathologies can lead to disorders of muscle activity in the form of spasticity and/or dystonia. In severe brain-injured, muscular hyperactivity easily gives rise to disabling outcomes that, added to other co-morbidities, leads to highly complex conditions: sometimes motor alteration is not compatible with postures that allow interaction with the environment; in some cases the very survival of the patient is put at risk by complications such as severe disorders of the urinary apparatus resulting from an overactive bladder. Orthopedic surgery, botulinum toxin, intrathecal baclofen, stimulators implanted in the basal ganglia are tools that can improve the quality of life of the patient significantly, if they are used keeping in mind the needs prevailing in each case.
