The above mentioned clinical case is being commented according to a cognitive perspective. The author, in discussing the article on a clinical case of a treatment to a patient with a diagnosis of Trichotillomania, offers a new possible understanding of the same clinical material. Particularly, the focus is posed both on the centrality of early experiences of exposure to critical, in the genesis and maintenance of the disorder; both on the metacognitive deficit which supports Livia’s alexithymic response to the environmental stress. The author suggests alternative indications for therapeutic practice, through a work on metacognitive deficit in the context of an individual psychotherapy. In conclusion, it’s suggested an integrated support of the approach EMDR, useful for reprocessing of traumatic experiences associated with themes of judgment, internal consistency and functionality of the behavior.
