The article explores the use of the metaverse and artificial intelligence (AI) in aim to enhance efficiency and effectiveness in “modern” territorial healthcare.The article discusses the challenges and opportunities in primary care within the context of healthcare reform, highlighting the increased workload for general practitioners due to bureaucratic procedures and a decline in the number of professionals.Therefore, the metaverse and AI aims to alleviate administrative burdens, allowing healthcare professionals to focus more on clinical care. The metaverse offers potential for telemedicine and remote training, while AI supports clinical decision-making. The paper, in this regard, emphasizes the importance of addressing issues related to privacy and the integration of health data. In conclusion, the pilot project at the “Casa di Comunità hub of Abbadia San Salvatore” (province Siena in Tuscany region), describes a practical application which integrates these technologies with current practice to improve access to healthcare services, as well as a new way of providing prevention, care and assistance.
