This paper presents a reflection on a clinical condition very well known to every psychotherapist but rarely researched, present in publications or presented in conferences. This work attempts to discuss the difference between patients who drop-out and therapeutic failures and to focus on these distinctions in relation to individual, couple and family therapy. In an attempt to rievaluate psychotherapy as a clinical science there has been an emergeing tendency in the last ten years to evaluate ways to avoid therapeutic failures.
