At the international level, organizational models for intermediate care services have been developing, as solution to emerging problem (aging, chronic disease, multiborbidity and frailty), to avoid inappropriate hospital admission and to delay institutionalization. The aim of the present study is to describe the case study of Emilia-Romagna Region to better define the role of "intermediate" services in long term care for Pearelderly. Therefore, data about facilities and services utilization from different Local Health Authorities of Emilia- Romagna Region, are collected from administrative databases to analyze intra-regional variability, main spill-over among health care settings, potential inappropriate care or gaps among services. From emerging evidence, the authors suggest a new framework for representing and planning intermediate care in the regional and national contest.
