The article is about research-training-intervention methodology applied to support Health Homes development in the Ferrara province. This methodology applied to health field combine Participatory Action Research and Permanent Education framework. Along the research muliti-disciplinar, intersectoral working groups were settled. Real case studies and specific tools were used to analyze health services delivery; evaluation and self evaluation about health services delivery and health professional competencies were fostered. Datas collected through the participative process pointed out the educational and training needs of the actors involved and group activities facilitated a peer educational process. Moreover these datas highlighted which organizational models could be adopted to support local health system transition.
