This article aims to investigate some aspects relating to liver transplantation in Italy. In particular, some historical notes will be exposed, the main causes of liver transplantation, the most recent epidemiological data compared to the 22 Italian Centers that carry out this intervention; the main indications and contraindications to transplantation will be presented and the organization of the Italian transplant network and the organ allocation criteria will be studied in depth. Finally, the topic of psychiatric comorbidities and abusive behaviors will be explored. This contribution also highlights the role of the bio-psycho-social complexity of the patient who is a candidate for hepatic transplantation, a complexity that is also reflected in a sustained care load both within and outside the hospital and which intensely engages the clinical activity of psychiatric consultation, for the which specific skills are required
