The article analyses important issues that characterised the Italian health system between 1945 and 1978. During the second post-war decades, there were numerous institutions involved in the Italian health system; they were characterised by overlapping competences and were ineffective in managing the provision of benefits aimed at the workers and their families. Through an exploration of the archival sources of the parties involved in citizens’ care, including ministries, public and private health insurance funds and hospitals, the essay seeks to explain the development of a significant debt in the general administration of healthcare. Those debts, stemming from a constant swaying of responsibility from one institution to another, from the poor management of the workers’ contributions, and from the extreme slow pace of the central institutions, remained unresolved until the creation of the Servizio sanitario nazionale (Ssn) in 1978.
