This action research intervention conducted in a community Mental Health Center (CSM) in Trentino (Italy) addresses the quality of information collected in psychiatric medical records by health care professionals. There is a widespread awareness of how heterogeneity in collecting and interpreting data is a critical aspect, leading to potential misunderstanding about meaning and relevance of treatment pathways. The primary aim of this study is to increase the efficacy of data sharing among people involved in mental health patient care. A more implicit but rele-vant aim is to improve the quality of available information in order to promote a more compe-tent response (diagnostic, rehabilitative, educational, therapeutic).
