This research aims to evaluate a model of patient care and treatment in terms of efficiency and effectiveness by observing the outcomes of health interventions in the management of patients with chronic conditions. Laying a solid foundation for the concrete improvement of the chronic patient requires greater involvement of general practitioners (GPs), who assume a primary role along the Patient Journey.The pursuit of these goals implies the use of supportive digital tools to adopt a data-driven approach to achieve greater integration between patient, hospital and territory.The research question is as follows: How well do chronic patients follow the therapies and suggestions of GPs? In order to answer this question, a quantitative methodology was used, collecting data through purpose-built questionnaires given to GPs to enter patient information.The research study produced results showing strong heterogeneity among the chronic conditions analysed with respect to therapy update and review.
