The paper deals with the theme of the technological innovation in medical sector, with particular reference to the projects of monitoring of the patients suffering from cardiac desease. The interpretative frame within which has been elaborated analyze the material of observation of case studies where were made an introduction of technological innovations (shared ICT tools). This particular point of view allows us to have a specific look regarding at least two aspects. The study of the organizations as place of knowledge set in evidence the different practices of representation and production of the knowledge and put different interlocutors of the organizations into the field research. Otherwise it is possible to tighten the field of investigation underlining in more explicit way the relationships established with the technology and the way of which people transform the practice and the same technology to reach different degrees of comprehension. More often in the everyday life the relationships are accompanied by technologies of various nature that change the relationships. They built other fillings and are represented by ourselves in different way according to the context we belong. Also in the case of the telemedicine this experience coming up more and more, especially with people with cardiac desease. The field of investigation concerns the comparison of two hospital centers that have started two separate projects beginning from the same premises and in the same times. The results of the job define the critical areas of these projects. Medical knowledge and computer science are normally not really integrated even if organizations and projects formally want it. These alignments, that organizations often exclude entirely, show the role of the organizations oriented to re-organize sequence of practices coming from different available knowledge setting to move towards a critical transition.
