This paper aims to draw from the technical and emotional richness of the clinical experience with parents and babies, to try to explore what happens in the therapeutic work with patients with severe difficulties from the early relational experiences. Infants, in the therapy room with their mothers, become patients to all intents and offer more valuable experience regarding the treatment of patients of different ages in particular characterized by severe withdrawal and autistic nuclei. Through the story and the deepening of some significant steps of the psychoanalytic therapy of a small child and a young man appears the possibility to reach and carry on the stage "inaccessible" states of mind, to give voice to the "always present in unconscious infant" as the unique possibility of representation of what had not yet access to the thinkable, as well as rewriting of past trauma with which it is impossible to live with.
