This paper focuses on the Attiv@bili Project - Innovazione Sociale e Assistenza Digitale in Ambienti Controllati - funded by Regione Lombardia. It aims at designing a new technological and organizational model for integrated care to frail people (Integrated Care - IC) at home or in assisted environments. Assistance at home foresees integrated social and healthcare to frail people along social and health assistance path. The use of ICT, such as Information Systems or Ambient Assisted Living, comprises platforms for interoperability among people through processes as well as domotics devices. Attiv@bili has focused the design phase based on a technological solution with integrated care and monitoring of care and health processes. This paper describes the design outcomes framing them in the state of the art of integrated assisted living.
