The authors present a case study of a patient - 32 years old, hospitalized in a high secure unit - that raises a few questions on the issue of "chronicity in psychiatry", such as: which patient is labelled "chronic" and which, on the other hand, is not yet labelled, but has a "destiny of chronicity"? Some features of the present case study could suggest a "destiny of chronicity": i.e. dual diagnosis, antisocial behaviour, a fragile family, pending legal problems, drug dependence and a network of services that has only recently started to collaborate. The authors present the anamnesis of the patient - that comprises key diagnostic elements and judicial information - followed by the description of the mother, who got the judicial process going through a complaint of violence and extortion of money. Two threads of reflection are then developed: Rocco’s history with drugs and his relationship with the law. Finally, the authors describe a network approach in the rehabilitation of "chronic" psychiatric patients, or of patients "with a destiny of chronicity", such as Rocco, and conclude the paper with open questions.
