Aim of this paper is to inspire the reader to think about the dilemma of keeping or revealing family secrets. Starting with the analysis of a clinical case, it tries to explain how both choices can coexist. While keeping the secret reflects the needs of self-protection of people who know it, revealing it to those who were previously unaware appears often inevitable for the patient as well as the family in order to find peace of mind. The two options will be considered as extremes of the same continuum. Case by case, the choice for one or the other is influenced by different factors which also determine the timing and circumstances.
