The study analyzes the evolution of personnel in the Italian National Healthcare Service (NHS) and the related public expenditure, resorting to NHS administrative data of the "Annual Financial Statement" (Conto Annuale) and the "Income Statement”"(Conto Economico) from 2001 to 2015. The analysis shows significant changes in the period 2001-2015, both as regards to the number of employees and the composition of the employee basis, especially with regards to four specific aspect: role, age, regional distribution, flexible staff. The analysis mainly highlights, a general decrease in workforce together with a higher expenditure up to 2010, when a reduction in wage dynamics and in spending began. The percentage of decrease in workforce was different between the Italian regions and between the professional groups. Moreover, the analysis brings out a trend of aging staff (the average age rose from 43.5, as it was in 2001, to 50.1 in 2015), and there is a statistical correlation between flexible staff and permanent staff only from 2011 to 2015. The authors therefore believe it is desirable to upgrade new planning tools in order to guarantee the correct turnover of NHS staff and the transfer of knowledge to younger workers. As regards to the management and training of the personnel, the continuous progress in treatment methods and technological innovation needs careful planning in the areas of training, work patterns, managerial behavior, and job satisfaction.
