In Italy, the SARS-CoV-2 viral pandemic impacts on a territorial organization of addictions widespread throughout the country. In the province of Bergamo there are 6 territorial services for addiction. National and regional regulations have defined the performance of addiction services as ESSENTIAL and, for this reason, during the most dramatic phase of the infection the services remained open, guaranteeing performance with an inevitable reorganization process. The work describes the organizational changes that the services had to implement, the impact of the infection on patients, what changes occurred in patients' access to services and how drug treatments with opioid agonists were managed.
