The paper analyses the role played by Regions and the Regional Health Services (Rhss) during and after the pandemic, focusing on two issues: the relations between the State and the Regions and the processes of convergence and divergence in the way the Rhss tackled the emergency. First, the author identifies three stages in the State-Regions relations between the years 2020 and 2022, with the outcome of an institutional strengthening both for the Regions and for the Central State. This raised difficult challenges in the governance of the Nhs. In the first wave of the pandemic, Regions adopted quite different solutions to the emergency, which were consistent and coherent with their different organisational and regulatory models. This was followed by a partial convergence of adopted solutions. These trends appear to be still present in the current re-organisation of hospital care and specifically in the implementation of the primary care reform. In time, this could lead to a deep change in the regulatory models adopted by some important Italian Regions, such as Lombardy.
