The author believes that the Ser.Ds (addiction services) play a key role within a global operating system where also NGOs belonging to the Civil Society considerably interact. Notwithstanding the "linear cuts" to the National Health System which progressively reduced staff and performances, the Ser.ds got credibility because of their close reference to scientific evidence and human rights which on the one hand influenced methods of intervention and on the other protected users from authoritarian measures. The continuous changes occurring in the consumption of psycho-active substances require the Ser.ds to review their strategies according to a new perspective where the healthcare dimension is included in the wider social issue with the support of education.
