This article studies the experience of Italian Hospitals of National Health System (NHS) on the degree of dissemination and maturity of the tools of CG. The authors analyzed the results of an online survey on 2022-2023 involving Hospitals and Local health Units joining to the Italian Federation of Healthcare Organizations (FIASO). The research uncovers both the presence of homogeneity in the adaptation to the organizational change of care and the lack of attention to issues related to the tools that support clinical activities: a) the encouragement of personalised treatment planning and the involvement of multidisciplinary teams; b) the need to focus on value based care and hospital-territory integration; c) the poor use of telemedicine and digital instruments able to track and monitor the patient in the various stages of his care path within the organization.
