The Italian psychiatric reform of 1978 marked a break with the idea of internment based on care and custody provided for in the 1904 legislation. Originally established as “Law 180” and included within the law establishing the National Health Service (SSN), the reform followed its subsequent events. Starting from a public policy perspective, this paper attempts to outline the features of mental health policies after the reform. It follows a timeline that takes into account the evolution of health policies from the early years, in which we witness the limits of the new public regulation of health care, to the last two decades of the 21st century, in which a system of regional regulation of the SSN has been established. The paper highlights some of the instruments of the policies that have contributed to determining the current structure of mental health services and to consolidating territorial differences that have existed since Law 180 was passed.
