This paper analyses the role of the Individual, Personalised, and Participatory Life Project as a cornerstone for promoting the inclusion and well-being of people with disabilities, particularly those facing multiple or layered vulnerabilities. Building upon the current legislative framework (Italian Law 328/2000; Legislative Decree 62/2024) and bio-psycho-social pedagogical paradigms, the study highlights how a holistic and person-centred approach can overcome the fragmentation of welfare systems, which remain excessively sectoral and disjointed. This paper presents research, conducted by the Kore University of Enna within the scope of the project “Let's make it a kaleidoscope: multiplying the places of inclusion!”. The findings confirm the persistence of structural weaknesses in the coordination of interventions, with particular impact on migrant families, while simultaneously emphasising the potential of the Educational Polyperipheries model as a pedagogical infrastructure capable of integrating diverse actions and stakeholders. From this perspective, the Life Project is positioned as a generative mechanism for inclusive processes, aimed at strengthening self-determination, co-design, and quality of life
