The increase in cronical pathologies and in disabilities, together with the increased life expectancy, have changed the epidemiological landscape of health needs in Italy. To cope with this different scenario home care delivery programs were launched some years ago. Unfortunately, the integration between primary care services, social service and home care have been accomplished only partially and therefore we can state that the community and the hospital are still two worlds apart. In the last years, the General Pratictioner has been asked for a huge burden of work, often at the crossroad of health promotion and social service. In order to increase the overall quality of health care a greater use of telemedicine and e-care is requested, expecially among GPs.
