This paper proposes both clinical and theoretical reflection about a method of intervention on work groups as a key element in organizational development. Furthermore, the authors emphasize the need of a dedicated operational and epistemological monitoring on the risk, which is always present in organizational contexts, of making the health establishment "institutional". These reflections are based on the experience gained by the Regional Reference Center of Critical Relations of the Region of Tuscany - Italy, with individual, groups and organizational consultancy within the health system.
