The local healthcare system of Lombardy, in the North of Italy, has recently introduced some relevant policies aimed at innovating the institutional and organizational framework of its health and care services and addressing chronic disease management. However, the final organizational arrangement of the Lombardy chronic care model will depend on the involvement of local primary care actors, the role played by public and private healthcare organizations and the collaborative relationships/interactions among the actors involved. The aim of this work is to describe the main undetermined dimensions of the model, to analyse the main institutional and organizational challenges related to the reform and to suggest policy recommendations. This could help policy makers implement the targeted measures needed to solve these rising issues.
