The present paper describes a schizophrenic delusion case, where treatment was combined with the modular approach (Lorenzini, Coratti, 2008) and the cognitive approach (Beck et al., 2011); considerations on the drop out outcome allowed to analyze some mistakes that, being reinterpreted through the systemic approach (Napier, Whitaker, 1981), highlighted the little importance given to schizophrenic family dynamics, thus creating a triangle among the therapist, the patient and his mother. On the other hand, trough a cognitive-evolutionist reading of the interpersonal motivation systems (Liotti, Monticelli, 2008), the analysis of therapeutic alliance allowed to understand which one played an essential role in therapy dynamics.
