The autor, describing a clinical case, questions the effects of the so-cial, cultural and psychic upheavals of today that have fared worse be-cause of the viral pandemic. The trend of psychotherapy before and after the onset of the spread of the infection is shown, followed by the limitation linked to confinement and then the vaccination campaign encouraged by the institution of the green card. The therapist is also involved in pathogenic dynamics in which reality forcefully enters the therapeutic process risking to change the rules of the setting. The pa-tient regresses to the schizoparanoid position and develops delusional symptoms of a persecutory type by marking massive use of defense mechanisms such as splittig, denial, projecton and projective identifica-tion. The author finally explores the theme of paranoia, a pathology that risks escaping diagnostic systems, which is often in the general population and shows itself in particulary traumatic moments.
