In this article, we made a comparison between the Sicilian and the Mafia institution of family, starting from the family myth of the union shared by Sicilian families’ members, a true mandate of loyalty, which overcomes distances and defines the belonging to a certain family and the identity of the individual members. We also tried to define some psychopathological outcomes of impossible junction processes, here defined as “pathologies of silence”, with strong values on corporeality. In order to explain this idea supported by the literature, we dedicated a part of this article to two clinical cases that exemplify the difficulty and suffering of individuals who find themselves struggling and betraying the family mandate, trying to respond to their evolutionary needs of autonomy, independence and unrecognized growth, in a continuous “conflict of loyalty”.
