This paper examined institutional maltreatment in out-of-home care systems, highlighting the tension between protective functions and the production of harm. Drawing on a framework that integrates experiential and professional knowledge, it proposed a multi-level conceptualization (micro, meso, macro) and a dynamic model of how harm is produced, normalized, and (not) recognized. The paper emphasized institutional responsibility and identified institutional courage as a key direction for transformative responses.
